Notice of Private Practice

EEB Therapy and Wellness is a private pay mental health practice.

This practice does not participate with insurance companies. Clients are responsible for the full cost of services at the time they are provided.

In some cases, clients may choose to submit documentation to their insurance provider for out-of-network reimbursement, depending on their individual insurance plan. However, reimbursement is not guaranteed and varies by provider and policy.

Choosing private pay therapy offers several benefits, including:

  • Increased privacy and confidentiality

  • Greater flexibility in treatment planning

  • Freedom from insurance limitations regarding diagnosis or session limits

 

Questions regarding fees, payment policies, or out-of-network reimbursement can be discussed during the consultation process.

Good Faith

Good Faith Estimate Notice

Your Right to Receive a Good Faith Estimate

Under the No Surprises Act, healthcare providers are required to give clients who are not using insurance an estimate of the expected costs for medical and mental health services.

At EEB Therapy and Wellness, clients who are paying privately have the right to receive a Good Faith Estimate explaining the anticipated cost of services.

What Is a Good Faith Estimate?

A Good Faith Estimate shows the expected charges for therapy services, including:

  • Individual therapy sessions

  • Consultation services

  • Any other reasonably expected services related to your care

The estimate outlines the projected cost of treatment for a specific period of time.

When You Will Receive Your Estimate

You have the right to receive a Good Faith Estimate:

  • When you schedule services with this practice

  • When you request an estimate before scheduling care

If you schedule services at least 3 business days in advance, you will receive your estimate in writing before your appointment.

 

If Your Bill Is Higher Than Your Estimate

If you receive a bill that is at least $400 more than your Good Faith Estimate, you have the right to dispute the bill.
